How to remove Eastham handle?

Unscrew the index button (part #1) counter-clockwise by hand, then pull it out.
Unscrew the set screw (part #2) counter-clockwise, using Philips or a flat screwdriver.
Pull out the lever handle (part #3).
Unscrew canopy (part #4) counter-clockwise by hand.
